The site navigation utilizes arrow, enter, escape, and space bar key commands. Left and right arrows move across top level links and expand / close menus in sub levels. Up and Down arrows will open main level menus and toggle through sub tier links. Enter and space open menus and escape closes them as well. Tab will move on to the next part of the site rather than go through menu items.

Medical Claim Procedures
  • For any incident or injury prior to September 1, 2021 please contact Jeff Schoonover.
  • For any incident or injury on or after September 1, 2021 please use the instructions below:
  1. Complete the online claim form using the link above.
  2. Once the claim has been submitted, Player’s Health will reach out to you directly. Claim forms should be submitted as soon as possible (within 60 days from date of injury). Player’s Health will then collect itemized bills, explanation of benefits (EOB’s) from your primary carrier, any cancelled checks for bills you have paid relating to this injury and/or any other necessary medical documentation.
